I've just attempted crafting it 4 times and it has failed 4 times. The probability of this happening 0.003429, or 0.3429%. What gives?

Bad luck

The chance that is displayed in the menu is the same chance you have of crafting the weapon, it uses the same algo. You can't change the crafting failrate without the menu changing accordingly.
