I've just attempted crafting it 4 times and it has failed 4 times. The probability of this happening 0.003429, or 0.3429%. What gives?

Bad luck
The chance that is displayed in the menu is the same chance you have of crafting the weapon, it uses the same algo. You can't change the crafting failrate without the menu changing accordingly.

If you hit 10/10 failed attempts then we might consider other possibilities.
But statistically, if you fail 10/10 attempts then you should succeed 65 times in a row after that
